The optimal response to neoadjuvant chemoradiation is the greatest strategy to provide
benefit to the bleak prognosis of locally advanced esophageal cancer (CEE). However, many do
not respond adequately, and to improve the therapeutic indexes of these we must know in
advance what patients will not obtain this benefit. Therefore, this review will synthesize the
possible molecular factors that could predict the response to neoadyuvant treatment.
The methodology consisted on searching studies in different databases with the stated
objective, having as a main endpoint the histopathological neoadjuvant response prediction. We
assign as main inclusion criteria patients with locally advanced CEEs and measurements of
markers performed in specimens of pre-neoadjuvant biopsies.
The results of the assessed studies showed significant association of being independent
predictive factors of response for the low expression of p53, p53R2, ERCC1, CD1, MT, VEGF,
COX-2, MT/p53, ERCC1/BRCA, high expression of BRCA1, cytoplasmatic MMS19,
CDC25B, Ki67 and + presence of “wild type p53" gene. The presence of combination of
certain miRNAs and genes also turned out to be independent response factors.
The molecular markers p53, ERCC1, BRCA1, p53R2, CDC25B, MMS19, MT, COX-2,
Ki67, CD1, various combinations of miRNAs and some genetic profiles are capable of response
prediction of neoadjuvat therapy. However, additional homogenized studies are needed to verify
these findings. [-]
La estrategia que mayor beneficio aporta al sombrío pronostico del cáncer de esófago
(CEE) localmente avanzado es la óptima respuesta a la quimiorradiación neoadyuvante. No

La estrategia que mayor beneficio aporta al sombrío pronostico del cáncer de esófago
(CEE) localmente avanzado es la óptima respuesta a la quimiorradiación neoadyuvante. No
obstante, muchos no responden adecuadamente y si queremos mejorar los índices terapeuticos
de estos debemos comenzar sabiendo con antelación que pacientes no obtendrán dicho
beneficio. Por ello, en esta revisión sintetizaremos los posibles factores moleculares candidatos
de predecir la respuesta en pacientes tratados con intención preoperatoria.
La metodología constó de la realización de una búsqueda de estudios en diferentes bases
de datos con el objetivo planteado, teniendo como variable principal la predicción de la
respuesta histopatológica neoadyuvante. Asignamos como principales criterios de inclusión
pacientes con CEEs localmente avanzados y medición de marcadores realizadas en especímenes
de biopsias pre-neoadyuvancia.
De los estudios evaluados, los resultados significativos de ser factores independientes
predictores de respuesta fueron la baja expresión de p53, p53R2, ERCC1, CD1, MT, VEGF,
COX-2, MT/p53, ERCC1/BRCA alta expresión de BRCA1, MMS19 citoplasmática, CDC25B,
Ki67 y presencia + gen
“wild type p53”
. La presencia de combinación de ciertos miRNAs y
genes también resultaron ser factores independientes de respuesta.
Los marcadores moleculares p53, ERCC1, BRCA1, p53R2, CDC25B, MMS19, MT,
COX-2, Ki67, CD1, varias combinaciones de miRNAs y perfiles genéticos, han demostrado
capacidad predictora de respuesta a la neoadyuvancia. Sin embargo, son necesarios estudios
homogeneizados adicionales para confirmar estos resultados. [-]
