Systemic lupus erythematosus is an autoimmune disease that clinically
exhibits
periods
of activity and remission. In Spain, its prevalence is estimated
to be
9 per 10,000
inhabitants, affecting mainly women of
childbearing age. Going into the relationship
between SLE and pregnancy, on the one hand, it has been observed that pregnancy
and postpartum are periods in which there is an increased risk of exacerbation of lupus
activity. On the other hand, the pregnant
woman with SLE has a higher risk of
complications than a healthy woman (fetal deaths, preeclampsia, preterm delivery ...).
For all these reasons, a great preconceptional and gestational control is necessary with
a programmed and multidisciplinary clinical
and analytical follow
-
up between the
specialist in obstetric medicine and rheumatology.
The present Final Degree Project has consisted in the realization of a
clinical practice
guide
for the gynecology and obstetrics service of the University Hospital of L
a Plana.
This
guide
aims to standardize the adequate management of pregnant women with
systemic lupus erythematosus. To do this, a systematic search has been made of the
current scientific evidence on the subject, and then applied to our system and health

El lupus eritematoso sistémico es una enfermedad autoinmune que clínicamente cursa
con periodos de actividad y remisión. En España, su
prevalencia se estima en 9 por
10.000 habitantes, afectando principalmente a mujeres en edad fértil. Adentrándonos
en la relac
ión entre el LES y el embarazo, p
or un lado, se ha observado que el
embarazo y el postparto son periodos en los que se asocia un
mayor riesgo de
exacerbación de la actividad lúpica. Por otro lado, la embarazada con LES tiene
mayor riesgo de complicaciones que una mujer sana (muertes fetales, preeclampsia,
parto pretérmino...). Por todo ello, es necesario un gran control tanto preconc
epcional
como gestacional con un seguimiento clínico y analítico programado y multidisciplinar
entre el especialista en medicina obstétrica y reumatológica.
El presente Trabajo de Final de Grado ha consistido en la realización de un
a guía de
práctica clíni

para el servicio de Ginecología y Obstetricia del Hospital Universitario
de La Plana.
Esta guía
ti
ene el objetivo de estandarizar el adecuado manejo de la
embarazada con lupus eritematoso sistémico. Para ello, se ha realizado una búsqueda
sistemática de
la evidencia científica actual sobre el tema, para después aplicarla a
nuestro sistema y departamento de salud. [-]
