Background:
There is an increasing trend of performing total thyroidectomy amongst patients
diagnosed with several thyroid diseases, considering hypocalcemia as the most frequent
complication during postoperative recovery.
Objective
: To determine hypocalcemia inciden
ce and to identify potential pathological and
surgical factors, which may be related to a major risk of hypocalcemia during postoperative immediate period after total thyroidectomy, as well as establishing the foundations for future
studies.
Methods:
Obser
vational and retrospective clinical research study. 94 patients that underwent
total thyroidectomy were studied by Hospital Universitario de La Plana ENT department,
between December 2014 and January 2017. Subclinical hypocalcemia was determined as 8,5
mg/
dl corrected calcium lower levels and clinical hypocalcemia when appearance of compatible
symptomatology.
Results:
Subclinical hypocalcemia was observed in 21 (22,3%) patients and clinical
hypocalcemia in 11 (11,7%) patients. Lower intraoperative iPTH lev
els were statistically
significant associated to a higher subclinical and clinical hypocalcemia. Hyperfunctioning thyroid
disease was statistically significant related with lower calcium levels. Patients who suffered from
hypocalcemia symptoms showed on av
erage an older age.
Conclusions:
Hypocalcemia is a common total thyroidectomy surgery complication.
Intraoperative iPTH, hyperthyroidism and older age are factors associated with a major risk of
hypocalcemia. No statistical significant associations with ot
her factors studied were identified. [-]

Existe una creciente tendencia a realizar tiroidectomías totales ante múltiples
patologías de tiroides, siendo la complicación más frecuente la aparición de hipocalcemia en el
postoperatorio.
Objetivos:
Determinar la incidencia de hipocalcemi
a e identificar los factores
patológicos y
quirúrgicos, que pudieran estar asociados a un mayor riesgo de hipocalcemia en el
postoperatorio inmediato de tiroidectomías totales, además de sentar las bases para futuros
estudios.
Material y métodos:
Estudio d
e investigación clínica observacional y retrospectivo. Se
estudiaron a 94 pacientes tiroidectomizados por el Servicio de Otorrinolaringología del Hospital
Universitario de La Plana, entre diciembre de 2014 y enero de 2017. Se definió hipocalcemia
subclínic
a como valores de calcio corregido inferiores a 8,5 mg/dl e hipocalcemia clínica
cuando además se presentaba clínica compatible acompañante.
Resultados:
Obtuvimos 21 (
22,3%) casos de
hipocalcemia subclínica y 11 (11,7%) casos de
hipocalcemia clínica.
Los n
iveles de PTH

i intraoperatoria bajos se asociaron a una mayor
incidencia de hipocalcemia subclínica y clínica.
La patología tiroidea hiperfuncionante se
relacionó significativamente con niveles de calcio inferiores. Los pacientes con hipocalcemia
clínica
presentaron signficativamente de media una mayor edad
.
Conclusiones:
La hipocalcemia es una complicación frecuente de las tiroidectomías totales.
Niveles bajos de PTH

i intraoperatoria, el hipertiroidismo y una mayor edad son factores
asociados a mayor rie
sgo de hipocalcemia. No se han encontrado relaciones significativas con
otros factores estudiados. [-]
