La esclerosis múltiple (EM) es una enfermedad degenerativa del Sistema Nervioso Central (SNC) caracterizada por lesiones desmielinizantes en el cerebro y en la médula espinal. El déficit cognitivo se considera uno de los síntomas más comunes e incapacitantes de la enfermedad. Frecuentemente, la velocidad del procesamiento de la información, el aprendizaje y la memoria, y las funciones ejecutivas representan las funciones cognitivas más afectadas durante el progreso de la EM. Estos déficits, junto a la depresión y la fatiga, tienen un gran impacto negativo en el desempeño de las actividades de la vida diaria y consecuentemente, sobre la calidad de vida de los pacientes. Por lo tanto, es importante investigar qué tipo de tratamientos son más eficaces. En este trabajo, se revisa la investigación más relevante sobre la rehabilitación cognitiva de la EM en los últimos cinco años. Recientemente, se han aplicado técnicas de neuroimagen para examinar los cambios producidos después de una intervención cognitiva. Parece ser que todos aquellos pacientes que han sido sometidos a rehabilitación presentan un aumento de la conectividad cerebral. Los hallazgos sobre la eficacia del tratamiento cognitivo en EM son prometedores, sin embargo, no se puede sacar conclusiones definitivas, debido, en gran parte, a la falta de rigor metodológico de los estudios. En futuras líneas de investigación, deberían contar con muestras de mayor tamaño y más homogéneas, incorporar un grupo control con participantes sanos y, valorar los cambios a nivel conductual, emocional, cerebral y sobre la calidad de vida de los pacientes EM. Multiple sclerosis (MS) is a degenerative disease of the Central Nervous System (CNS) characterized by demyelinating lesions in the brain and spinal cord. One of the most common and disabling symptoms of the disease is cognitive deficit. Commonly, the speed of information processing, learning and memory, and executive functions represent the cognitive functions most affected during the MS development. These deficits, together with depression and fatigue, have a great negative impact on the performance of daily living activities and, consequently, on the quality of the patients’ life. Therefore, it is important to investigate which type of treatments are most effective. In this paper, we review the most relevant research on the cognitive rehabilitation of MS in the last five years. Recently, neuroimaging techniques have been applied to examine changes following cognitive intervention. It appears that all those patients who have undergone rehabilitation have an increase in cerebral connectivity. Findings on the efficacy of cognitive therapy in MS are favorable, however, no definite conclusions can be drawn, largely due to the lack of methodological rigor of the studies. In future researches, they should have larger and more homogeneous samples, incorporate a control group with healthy participants, and assess behavioral, emotional, and cerebral and quality of life changes in MS patients.
