Introducción: la Enfermedad Inflamatoria Intestinal Pediátrica (EIIP), cuya incidencia está
aumentando, presenta peculiaridades que la diferencian de la forma adulta, por lo que requiere
un abordaje específico.
Objetivos: elaborar un protocolo para el diagnóstico, tratamiento y seguimiento de la EIIP
adaptado al Hospital General de Castellón.
Metodología: se realiza una búsqueda bibliografía en: Pubmed, Biblioteca Cochrane Plus, Up
to date, páginas web de asociaciones médicas españolas, europeas y americanas, revistas y
libros de consulta.
Resultados: el diagnóstico debe combinar clínica, pruebas de imagen, endoscopias, laboratorio
y microbiología. La radiación debe evitarse siempre que sea posible. Se propone un algoritmo
de tratamiento para cada patología. Presentamos unas tablas con los posibles tratamientos a
emplear, posología, efectos adversos y preparaciones farmacológicas. Se destaca el uso precoz
de tratamiento biológico (top down approach) y el ahorro de corticoides. Respecto al
seguimiento, enumeramos una serie de recomendaciones para evitar el retraso de crecimiento y
puberal, la desnutrición, la infección por gérmenes oportunistas o reactivaciones, secundarias a
fármacos inmunosupresores, y vigilar la aparición de manifestaciones extraintestinales.
Conclusiones: serán necesarios un diagnóstico precoz, un tratamiento óptimo y una vigilancia
estrecha para lograr el adecuado control de la enfermedad. [-]

Background: Pediatric Inflammatory Bowel Disease (PIBD), which presents an increasing
incidence, has some peculiarities that differentiate it from the adult form and therefore requires
a specific approach.
Objectives: the main objective is to develop a protocol for the diagnosis, treatment and followup
of PIBD adapted to the Castellon General Hospital.
Methodology: a bibliographic search is carried out in: Pubmed, Cochrane Library Plus, Up to
date, Spanish, European and American medical associations websites, journals and reference
books.
Results (Key points): the diagnosis must combine clinical, imaging, endoscopy, laboratory and
microbiology. Radiation should be avoided whenever possible. A treatment algorithm is
proposed for each pathology. We present tables with possible treatments to be used, posology,
adverse effects and pharmacological preparations. Highlight the early use of biological
treatment (top down approach) and steroid saving. With regard to follow-up, we list a number of
recommendations to avoid delayed growth and puberty, malnutrition, infection by opportunistic
germs or reactivations, secondary to immunosuppressive drugs, and look out the appearance of
extraintestinal manifestations.
Conclusions: an early diagnosis, an optimal treatment and a close monitoring are necessary to
achieve an adequate control of the disease. [-]
