El cáncer de páncreas localmente avanzado (LAPC) irresecable asocia un
pronóstico infausto, siendo la quimiorradiación la mejor opción terapéutica.
Recientemente, se ha valorado la radioterapia estereotáctica extracraneal (SBRT)
como alternativa. Se realizó una revisión sistemática para evaluar la eficacia y
toxicidad de las modalidades terapéuticas, insistiendo en la SBRT.
Materiales y métodos
Se realizó una búsqueda bibliográfica en distintas bases de datos. La
supervivencia global fue la variable principal. La supervivencia libre de progresión,
tiempo hasta la progresión, respuesta clínica, tiempo medio de supervivencia y efectos
adversos fueron las variables secundarias.
Resultados
Únicamente un artículo cumplió los criterios de selección: una revisión de 26
ensayos clínicos aleatorizados, con 6.673 pacientes. El no disponer de los datos
brutos de los ensayos limitó la comparación de resultados. No obstante, la SBRT
demuestra importantes beneficios, pues proporciona dosis elevadas de radiación
ablativa en un intervalo reducido de tiempo, evitando las interrupciones derivadas del
tratamiento sistémico. También proporciona alivio sintomático e incrementa los índices
de resecabilidad quirúrgica. Además, muestra una toxicidad gastrointestinal aceptable.
Conclusiones
La SBRT tras quimioterapia de inducción para el LAPC irresecable es factible y
tolerable, logrando beneficios clínicos de interés. Sin embargo, se requieren estudios
prospectivos randomizados adicionales. [-]

Unresectable locally advanced pancreatic cancer (ULAPC) is associated with a
negative prognosis, being chemoradiotherapy the best therapeutic option. Recently,
stereotactic body radiation therapy (SBRT) has been considered as an alternative. We
performed a systematic review to evaluate the efficacy and toxicity of therapeutic
modalities, insisting on SBRT.
Materials and methods
We searched for publications on different databases. Overall survival was the
primary endpoint that we evaluated. Progression-free survival, time to progression,
clinical response, median survival and adverse effects were the secondary endpoints.
Results
Only one article met the selection criteria: a systematic review including 26
randomized clinical trials, with 6.673 patients. The fact of not having raw data from
clinical trials made it difficult to compare results. However, SBRT shows important
benefits as it provides high doses of ablative radiation in a reduced time interval,
avoiding interruptions that follow systemic treatment. It also provides symptomatic relief
and increases surgical resectability rates. In addition, it shows an acceptable
gastrointestinal toxicity.
Conclusions
SBRT after induction chemotherapy for ULAPC is feasible and tolerable,
achieving interesting clinical benefits. Additional randomized prospective studies are
nonetheless required to grant such conclusion. [-]
