Objective: We explored the neural efficiency hypothesis in cognitively preserved multiple sclerosis (MS) patients. To observe brain activation for fast and slow information processing speed (IPS) requirements, we used an adapted version of the Symbol Digit Modalities Test (SDMT) with different inter-stimuli intervals (ISI).
Methods: 16 MS patients and 17 healthy controls (HC) were enrolled. All the participants were neuropsychologically assessed and did the SDMT adapted for functional Magnetic Resonance Imaging (fMRI) with different ISI presentations (1.5, 2 and 2.5 s).
Results: We observed differences between patients and HC while performing the SDMT at 1.5 sec. but not for the 2 and 2.5 ISI presentations. The MS patients showed increased brain activation while performing the SDMT at 2.5 and 2 s ISIs. The regression analysis between SDMT performance for each ISI condition and brain activation showed a positive correlation in the MS patients between the behavioral execution of the SDMT for the 1.5 s and 2 s ISI conditions and frontal brain areas. However, a negative correlation was found between the 1.5 s ISI performance and frontal areas for HC.
Conclusions: According to the neural efficiency hypothesis, slower individuals (MS patients) showed increased activations compared to faster individuals (HC) while performing the SDMT. Using an adapted version of the SDMT with a superimposed shorter ISI presentation (1.5 s), IPS deficits were manifested in the MS patients, which were cognitively preserved. [-]

Observamos diferencias entre pacientes y CS al realizar el SDMT en la presentación de 1.5 seg. pero no en las presentaciones de IIE de 2 y 2.5. Los pacientes con EM mostraron un incremento en la activación cerebral mientras realizaban el SDMT con IIE de 2.5 y 2 seg. Los análisis de regresión entre el desempeño en cada condición del SDMT y la activación cerebral mostraron una correlación positiva en los pacientes con EM entre la ejecución conductual del SDMT en las condiciones de IIE de 1.5 y 2 seg. y activación en áreas frontales. Sin embargo, en los CS se encontró una correlación negativa entre el desempeño en la condición de 1.5 seg. y activación en áreas frontales.
Conclusiones: De acuerdo con la hipótesis de eficiencia neural, los individuos más lentos (pacientes con EM) mostraron un incremento en la activación cerebral en comparación con los individuos más rápidos (CS) durante la realización del SDMT. Al utilizar una versión adaptada del SDMT con una presentación impuesta de IIE más breve (1.5 seg.), se manifestaron déficits en la velocidad del procesamiento de la información en pacientes con EM que se encontraban cognitivamente preservados.
