Antecedentes: La enterocolitis necrotizante (NEC) se considera la patología intestinal adquirida más grave y frecuente del periodo neonatal, con una incidencia del 90% en recién nacidos prematuros (RNPT). Su etiología es multifactorial y se caracteriza por alteración intestinal tras una colonización patógena bacteriana que desemboca en necrosis del epitelio gastrointestinal, presentando alta morbimortalidad en pretérminos. No obstante, se plantea la hipótesis que la alimentación del prematuro con lactancia materna exclusiva (LME) disminuye el riesgo de desarrollar NEC. Objetivos: Se establece como objetivo principal determinar la relación entre la LME y la NEC en RNPT y, más específicamente, la disminución de su tasa de desarrollo. En correlación, también se objetivizan los beneficios e inconvenientes de este tipo de alimentación. Metodología: Estudio observacional cualitativo a través de una revisión integradora de la literatura con alto contenido de evidencia científica, seleccionando únicamente ensayos clínicos y estudios multicéntricos publicados en los últimos 5 años. Se establece población diana de RNPT <37 semanas de gestación (SG). Resultados: Existe una disminución en las tasas de NEC en RNPT alimentados con LME, presentando porcentajes inferiores al 20% en 83,4% (n=10) e inferiores al 10% en 41,7% (n=5) del total de 12 artículos. Conclusión: Parece ser que la LME es el mejor método preventivo para el desarrollo de NEC en RNPT. No obstante, se precisa de nuevas investigaciones más exhaustivas en el ámbito clínico español para reafirmar con certeza dicha teoría evidenciada en otros países. [-]

Background: Necrotizing enterocolitis (NEC) is considered the most severe and acquired intestinal disease and it frequently occurs in the neonatal period, with an incidence of 90% in preterm infants. The etiology is multifactorial and is characterized by impaired intestinal bacterial pathogen colonization which produces necrosis in the gastrointestinal epithelium, showing high morbidity in preterm. However, the hypothesis suggests that early feeding with exclusive breastfeeding decreases the risk of developing NEC. Aims: The main objective is to determine the relationship between the exclusive breastfeeding and the NEC in preterm infants and, more specifically, the decrease of its development rate. Moreover, the benefits and disadvantages of this type of food are also studied. Methodology: Qualitative observational integrative literature review with high scientific evidence, selecting only clinical trials and multicenter studies published in the last 5 years. Target population of preterm infants <37 weeks gestation (SG) is established. Results: There is a decrease in rates of NEC in preterm infants fed exclusive breastfeeding, having percentages less than 20% in 83.4% (n = 10) and less than 10% in 41.7% (n = 5) from 12 items. Conclusion: It seems that the exclusive breastfeeding is the best preventive method for the development of NEC in preterm infants. However, it requires an updated and extensive research in the Spanish clinical setting to reaffirm the theory which has been evidenced in other countries. [-]
