L’epilèpsia del lòbul temporal (ELT) sol tindre origen en l’hipocamp, regió que esdevé atrofiada produint pèrdua de memòria. Ja que gran quantitat de pacients amb ELT no respon al tractament farmacològic, la cirurgia acaba sent l’única opció. Aquest estudi longitudinal es va centrar en dos grups de pacients amb ELT dret (ELTD; n=14) i ELT esquerre (ELTE; n=15), que foren avaluats abans i després de la intervenció a nivell neuropsicològic i anatòmic (MBV) amb l’objectiu de conèixer l’impacte de la cirurgia en l’epilèpsia per a cada grup. Els resultats neuropsicològics no van mostrar una afectació significativa de la memòria post-cirurgia. Les comparacions inter-grup mostraren pitjor execució per al grup ELTE en el Record a curt termini, mentre que la interacció Temps-Grup va tindre efecte sobre el test de denominació Boston, empitjorant l’execució post-cirurgia per a ELTE. Els resultats anatòmics, mostraren pèrdua de volum extra-hipocampal ipsilaterals per a ambdós grups: tàlem i insula per a ELTD i cap del caudat per a ELTE. La pèrdua de substància gris (SG) del caudat va correlacionar amb el canvi en l’execució del test Boston. Finalment, l’anàlisi de regressió del lòbul temporal pre-quirúrgic amb els canvis neuropsicològics obtingué una correlació significativa per a ELTE entre el canvi del Record a llarg termini i el volum de SG del hipocamp dret. Dels petits canvis neuropsicologics es pot concloure que els controls pre-quirúrgics duts a terme foren exitosos. La pèrdua de volum en SG fou localitzada per a ambdós grups en regions funcional i/o anatòmicament connectades a l’hipocamp extret, explicant alguns dèficits de memòria: tàlem relacionat amb memòria general, insula amb meta-memòria i caudat amb denominació. La rellevància del hipocamp dret en el Record a llarg termini va ser explicada per una redistribució cerebral de les funcions deguda a l’extracció de la part homologa dreta. [-]

Temporal lobe epilepsy commonly has its epileptogenic focus in hippocampus, which becomes atrophied over time producing memory decline. Since several patients with TLE do not respond to pharmacological treatment, the resection of the affected part of hippocampus is considered the best option. This longitudinal study was focused on two groups of patients with right (RTLE; n=14) and left TLE (LTLE; n=15), who were pre and post-surgical evaluated with a neuropsychological battery and anatomically through voxel-based morphometry in order to knowing the impact of TLE surgery for each group. Neuropsychological results did not show significant general memory impairment after surgery, although some specific patients did. Between-group comparisons showed more impairment in LTLE than RTLE for Short-delay free recall and the interaction Time x Group had an effect on Boston vocabulary naming test showing a decline on performance after surgery for LTLE. Regarding to anatomical results, extra-hippocampal gray matter loss was found after intervention being ipsilateral for both groups: thalamus and insula for RTLE and the head of the caudate for LTLE. The GM loss in the caudate correlated with change of performance in Boston Naming Test. Finally, pre-surgical temporal lobe regression analyses with neuropsychological changes found a significant correlation for the LTLE group between the Long-delay Free Recall change and the GM volume in right hippocampus. Pre-operational clinical control for our sample was successfully carried over since only small neuropsychological changes were found. Gray matter volume loss was located for both groups on regions functionally and/or anatomically connected to the resected hippocampus, explaining some memory deficits: thalamus related to general memory, insula to meta-memory and caudate to naming. Right hippocampus relevance for Long-delay free recall was explained by the brain redistribution of functions due to resection of the left counterpart. [-]
