Consumo de alcohol, síndrome metabólico y desarrollo de diabetes tipo 2

Abstract

This review is based on the existing controversy ab out the influence of alcohol on the development of metabolic syndrome (MS) and type 2 d iabetes (T2D). Objective : To synthesize and analyze the reviews, systematic reviews and met a-analyzes that relates consumption of alcohol and the risk of developing T2D. Methodology : A systematic research was undertaken in PubMed and Scopus of articles published in Engli sh and Spanish between January 2008 and December 2017, which will analyze the risk of devel oping MS and T2D associated with alcohol consumption. The quality of the articles was evalua ted with the tool SIGN (Scottish Intercollegiate Guidelines Network). Results : 2321 studies were found, among which 26 were finally included in the review. These studies seem to indicate that moderate consumption plays a protective role in the development of MS and T2D, contributing to a better profile of insulin sensitivity (IS). This protective role could vary d epending on the type of alcoholic beverage, as well as gender and race. Conclusions : Although it seems that high levels of alcohol consumption increase the risk of T2D and that light and moderate levels decrease it, the available evidence is not robust, being necessary m ore rigorous studies and with standardized categories of consumption.

Esta revisión se fundamenta en base a la controvers ia existente sobre la influencia del alcohol en el desarrollo de síndrome metabólico (SM) y diabete s tipo 2 (DT2). Objetivo : Sintetizar y analizar las revisiones, revisiones sistemáticas y meta-análisis que relacionen el consumo de alcohol y el riesgo de desarrollar SM y DT2. Metodología : Se realizó una búsqueda sistemática en PubMed y Scopus de artículos publicados en inglé s y español entre enero de 2008 y diciembre de 2017 que analizaran el riesgo de desar rollo de SM y DT2 asociados al consumo de alcohol. Se evaluó la calidad de los artículos con la herramienta SIGN (Scottish Intercollegiate Guidelines Network). Resultados : Se encontraron 2321 estudios, de los cuales final mente se incluyeron 26 en la revisión. Dichos trabajos parec en indicar que un consumo moderado ejerce un papel protector sobre el desarrollo de SM y DT2, contribuyendo a un mejor perfil de sensibilidad a la insulina (SI). El consumo elevado sin embargo parece ejercer como factor de riesgo. El papel protector podría variar en función del tipo de bebida alcohólica, así como del género y la raza de los consumidores. Conclusiones : Aunque parece ser que niveles elevados de consumo de alcohol aumentan el riesgo de DT2 y que niveles ligeros y moderados lo disminuyen, la evidencia disponible no es robusta, siendo necesarios estudios más rigurosos y con categorías estandarizas de consumo.
