Plasma rico en plaquetas y artrosis de rodilla, ¿En qué punto nos encontramos en la actualidad?

Abstract

Introduction: nowadays, the knee osteoarthritis in one of the diseases with highest prevalence globally. In order to improve its management and conservative treatment, the investigation of a treatment based on regenerative medicine has recently emerged: the Platelet-Rich Plasma (hereafter, PRP). Objectives: the aim of this literature review is to evaluate the effectiveness of such treatment in patients with knee osteoarthritis along the time. Methodology: the present paper includes 13 of the latest randomized clinical trials (hereafter, RCT), recruited from a structured research of Pubmed and Cochrane. As for the synthesis of the results, a results chart has been created, where the scores achieved are unified in scales or questionnaires, which evaluate the pain, physical function and the quality of life, as well as the quality of the methodology used in each publication. Results: out of the total studies recruited, 11 papers achieved higher scores than the groups treated with PRP (in comparison to the control groups), in 3, 6 and 12 months post-treatment. Conclusion: Due to the heterogeneity of the papers included, as well as the existence of important methodological bias, it is not possible to evidently conclude that the injections of PRP are effective as osteoarthritis treatment.

Introducción: en la actualidad la artrosis de rodilla es una de las enfermedades con más prevalencia a nivel mundial. Con el fin de mejorar su manejo y tratamiento conservador, recientemente se ha empezado a investigar un tratamiento basado en medicina regenerativa, el Plasma Rico en Plaquetas (PRP). Objetivos: la finalidad de esta revisión es evaluar la eficacia de dicho tratamiento en el tiempo, en pacientes con artrosis de rodilla. Metodología: la revisión incluye 13 de los últimos ensayos clínicos aleatorizados (ECAs), reclutados a partir de una búsqueda estructurada de Pubmed y Cochrane. Para la síntesis de los resultados se elaboró una tabla resumen en la que se unificaron las puntuaciones obtenidas en escalas o cuestionarios, que evaluaban el dolor, la función física y la calidad de vida, analizándose además la calidad metodológica de cada publicación. Resultados: del total de estudios reclutados, en 11 ensayos se obtuvieron mejores puntuaciones, en los grupos tratados con PRP, comparado con los grupo control, a los 3, 6 y 12 meses post- tratamiento. Conclusión: debido a la heterogeneidad de los trabajos incluidos, así como a la existencia de importantes sesgos metodológicos, no se puede concluir de manera evidente que las inyecciones intraarticulares de PRP sean efectivas como tratamiento de la artrosis.
