Limitación del esfuerzo terapéutico (Let) y su aplicaicón entre los profesionales de enfermería del Hospital Provincial de Castellón.

Abstract

Introduction: Limitation of Therapeutic Effort (LET) is a common practice in different hospitals in Spain and othe r Western countries when there is no clinical cure in a patient. However, there is great social and professional confusion between different concepts such as LET, euthanasia (active / passive), palliative care, sedation, etc., and an obvious lack of criter ia and even secrecy, reserve or some cover - up at the time of applying it, among the different health professionals. Objectives: To know the range of abilities and powers of the current knowledge of the nursing professionals who develop their work in the H ospital Provincial in Castellón in the following areas: Internal Medicine, Oncology, Household Hospitalization Unit (UHD) and Intensive Care Unit (ICU). Methodology: A sample of 36 nurses selected from the hospital has been analyzed, who are in direct and daily contact with terminal / palliative patients, and who have a high knowledge of the care of this type of pathologies; they were subjected to an own elaboration questionnaire with short questions, in order to obtain closed answers of the type: Yes / No . Results: the 65,7% of participants think that LET is whereas the retreat as the no establishment of measures, the 67% think that consists just in lending the patient a palliative care. On the other hand, the approval level that they have with LET's apply it has an average of 9,60 (about 10). Conclusions: although the participants have knowledge about LET it exists misunderstanding in some aspects.

Introducción: La Limitación del Esfuerzo Terapéutico (LET) es una práctica usual en los distintos hospitales de España y otros países occidentales cuando en un paciente no existe curación clínica. Sin embargo, existe una gran confusión social y también pro fesional entre conceptos diferentes como, la LET, la eutanasia (activa/pasiva), los cuidados paliativos, la sedación etc., y una evidente falta de criterio e incluso sigilo, reserva o cierto encubrimiento a la hora de aplicarla entre los diferentes profesi onales sanitarios. Objetivos: Saber hasta dónde alcanzan los conocimientos actuales sobre LET que tienen los profesionales de enfermería del Hospital Provincial de Castellón en las plantas de Medicina Interna, Oncología, Unidad de hospitalización domiciliaria (UHD) y Unidad de cuidados intensivos (UCI); y conocer su opinión sobre la LET. Metodología: Se han analizado una muestra de 36 enfermeras/os seleccionados del citado hospital, por estar en contacto directo y diario con el paciente terminal/paliativo, y por tener un alt o conocimiento del cuidado de este tipo de patologías, a quienes se les aplicó un cuestionario de elaboración propia con preguntas cortas. Resultados: el 66% de los participantes piensa que la LET es tanto la retirada como la no instauración de medidas y e l 67% cree que consiste en prestar únicamente al enfermo cuidados paliativos. Por otra parte, el nivel de conformidad que tienen con la aplicación de la LET es, de media, de 9,60 (sobre 10). Conclusiones: Aunque los participantes tienen conocimientos sobre la LET existe algo de confusión en ciertos aspectos.
