El trastorno depresivo es una de las enfermedades más prevalentes de todo el mundo. Además, es la primera causa psicológica de discapacidad y contribuye de forma importante a la carga mundial de comorbilidad. Actualmente, existen múltiples tratamientos farmacológicos, pero no resultan efectivos en todos los pacientes, lo que ha llevado a estudiar los procesos subyacentes que causan la depresión. En esta revisión, se pretende esclarecer el estado actual de la investigación sobre la depresión. En primer lugar, se establecerán las bases neuroanatómicas de la depresión, así como las consecuencias a nivel morfológico. Por otra parte, se revisará cuáles son los principales mecanismos en la etiología de la patología depresiva desde una clasificación exógena/endógena de la depresión, teniendo en cuenta factores biológicos, psicológicos y ambientales. En lo que se refiere al entorno, se tendrán en cuenta los factores ambientales que influyen en la vulnerabilidad al desarrollo de la patología depresiva, así como la interacción GxE. Por otra parte, en lo que respecta a los factores biológicos, actualmente, se ponen de manifiesto diferentes hipótesis sobre las causas de la depresión como, por ejemplo, la hipótesis monoaminérgica, la teoría sobre la desregulación neuroendocrina y las implicaciones del sistema inmunitario en la patofisiología de la depresión y la relación existente entre todas ellas. Por último, se revisará la actualidad en cuanto a los tratamientos farmacológicos utilizados y el futuro de los antidepresivos.Major depressive disorder is one of the most prevalent diseases worldwide. Furthermore, it is the first psychological cause of disability and a major contributor to the global burden of comorbidity. Currently, there are multiple treatment, but not effective in all patients, and this has led to study the underlying processes which cause depression. In this review, is to clarify the current state of research on depression. First, neuroanatomical basis of depression is established as well as consequences morphologically. Moreover, which will be reviewed are the main mechanisms in the etiology of depressive disorders from an exogenous/endogenous depression classification, considering biological, psychological and environmental factors. In regards to the environment, the environmental factors that influence vulnerability to the development of depressive disorders and the GxE interaction are taken into account. On the other hand, in regard to biological factors, at present, show different hypotheses about the causes of depression such as the monoamine hypothesis, the theory of the neuroendocrine dysregulation and implications on the immune system in the pathophysiology of depression. In addition we will discuss the relationship among them. Finally, will be reviewed regarding pharmacological treatments currently used and future antidepressants.
