Objectives:
To describe the results obtained with the application of NIMV in patients with
hypoxemic respiratory insufficiency caused by pneumonia, in the context of an HIV infection; as
well as looking for variables that correlate with the success/failure of the therapy.
Design:
Retrospective observational descriptive study (period 2006-2017) performed in medical-
surgical ICU.
Results:
A total of 15 patients with HIV infection and pneumonia were included in the study, who
required NIMV, recording 12 successes and 3 failures of the same, and presenting a low rate of
complications. The variables that were correlated in a statistically significant way with the
success/failure of the therapy were hepatitis, tuberculosis, blood pH at admission to the ICU,
duration of total MV (IMV+NIMV) and following complications: barotrauma, evolution to ARDS,
cardiac arrest, need for HF/HD, pneumothorax, pneumonia acquired in the ICU, UTI and
bacteremia.
Discussion:
In our service, NIMV has given very good results, with a small rate of both failures
and complications. Given the small sample size of the study, it is not possible to extrapolate the
results obtained to the general population without an important risk of error, but they will be useful
to propose future lines of research.
Objetivos:
Describir los resultados obtenidos con la aplicación de la VMNI en pacientes con
insuficiencia respiratoria hipoxémica causada por una neumonía, en el contexto de una infección
por VIH; así como buscar variables que se correlacionen con el éxito/fracaso de la terapia.
Diseño:
Estudio descriptivo observacional retrospectivo (período 2006-2017) realizado en UCI
médico-quirúrgica.
Resultados:
Se incluyeron en el estudio un total de 15 pacientes con infección por VIH y
neumonía que precisaron VMNI, registrándose 12 éxitos y 3 fracasos de la misma, y presentando
una baja tasa de complicaciones. Las variables que se correlacionaron de una forma
estadísticamente significativa con el éxito/fracaso de la terapia fueron la hepatitis, la tuberculosis,
el pH sanguíneo al ingreso en UCI, la duración de la VM total (VMI+VMNI) y las siguientes
complicaciones: barotrauma, evolución a SDRA, parada cardíaca, necesidad de HF/HD,
neumotórax, neumonía adquirida en UCI, ITU y bacteriemia.
Discusión:
En nuestro servicio, la VMNI ha dado muy buenos resultados, con una pequeña tasa
tanto de fracasos como de complicaciones. Dado el pequeño tamaño muestral del estudio no es
posible extrapolar los resultados obtenidos a la población general sin un importante riesgo de
error, pero serán útiles para plantear futuras líneas de investigación.
