El cáncer es una de las enfermedades que causa mayor número de muertes en nuestra sociedad.
El progreso en su tratamiento ha aumentado la supervivencia media de los pacientes afectados
por esta enfermedad y ha pasado a considerarse una enfermedad crónica, incrementándose el
interés por el estudio de los aspectos psicológicos y sociales asociados al cáncer. Esto es debido
en parte al observarse las limitaciones de la aplicación de un modelo asistencial estrictamente
médico.
Objetivo
Estudiar en la práctica clínica diaria el estado psicológico de los pacientes con cáncer, las
variables sociodemográficas y clínicas implicadas y su papel modulador en el estado
psicológico, así como los tratamientos psicológicos empleados.
Revisión de 20 historias clínicas de pacientes fallecidos por cáncer en el año 2013 y comienzos
del actual año 2014 que han sido previamente atendidos en la Unidad de Psicooncología del
Consorcio Hospitalario Provincial de Castellón.
Resultados
Alta prevalencia de problemas de salud mental (85%) y síntomas físicos (80%). La estrategia de
afrontamiento inicial ante la enfermedad más empleada fue el espíritu de lucha (35%). Una
minoría (20%) se apoyó en sus creencias religiosas, siendo la práctica más común entre ellos
rezar. El 95% de la muestra percibió un buen apoyo social durante su enfermedad. La
intervención psicológica se sustenta en la terapia cognitivo-conductual adyuvante. Se han
encontrado diferencias significativas en el estado psicológico en función del estilo de
afrontamiento adoptado; y en el estilo de afrontamiento entre hombres y mujeres.
Conclusiones
Es necesario seguir investigando en el futuro, para conseguir sintetizar la información de los
distintos estudios y lograr resultados concluyentes. Se deben mejorar los sistemas de detección
y evaluación de problemas de malestar emocional y salud mental de los pacientes oncológicos y
seguir promoviendo la coordinación del psicólogo con el resto del equipo médico.
Cancer is one of the diseases which causes the greatest number of deaths in our society. The
progress in its treatment has improved the average survival rate of patients affected by this
disease. This has been established as a chronic disease, what increases the interest in the studies
of psychological and social aspects related to cancer. This is so because it has been observed the
limits of a strictly medical healthcare model.
Objective
The aim is to study the psychological responses of cancer patients as well as all the clinical and
sociodemographic variables involved and its modulator role in the psychological responses.
Also the most used psychological treatments in the daily clinical practice.
Method
Revision of 20 medical histories of deceased patients of cancer in the year 2013 and early this
year 2014 previously assisted in the Psycho-Oncological Unit of Consorcio Hospitalario
Provincial of Castellon.
Results
High prevalence of mental health problems (85%) and physical symptoms (80%). The most
used strategy of initial coping with the disease was the fighting spirit (35%). A minority (20%)
leaned on religious beliefs, whose most common practice was praying. 95% of sample received
a good social support during their illness. Psychological intervention is based on the cognitive
behavioural adjuvant therapy and therapies are mainly based on three types of techniques:
behavioural and cognitive techniques and emotional expression. There are significant
differences in the psychological response depending on the strategy of coping used. Also there
are significant differences in the strategy of coping used between men and women.
Conclusions
It is necessary to carry out additional research in the future so that the information of different
studies can be synthesized and we can get conclusive results. Systems of detection and
evaluation of problems of emotional distress and mental health of cancer patients must be
improved. It is important to promote the coordination among the psychologist and the rest of
health team.
